(A) – KYU GRADE FORM
KYU Grading is a Student level grading Application form
There are 9 grades of elementary level before attaining black belt.
Which covers
White belt 9th kyu
Yellow belt 8th kyu
Orange belt 7th kyu
Green belt 6th kyu
Purple belt 5th kyu
Purple white 4th kyu
Brown 3rd kyu
Brown white 2 nd kyu
Brown red 1st kyu
These grades will be in descending order.
(B) – DAN GRADE FORM
DAN Grades are the professional section of
grading starts from first dan to
10th dan in ascending order
There are ten higher degrees
which will be awarded for the
proficiency they gain by practice,
Knowledge gained by teaching and
the support given to the institution
they belong to, by developing
themselves and the institution.
(C) – INDIVIDUAL MEMBERSHIP APPLICATION FORM
“Form for individual membership application “
This form can be used by all instructors while enrolling new admission to their class.
This should be duly filled in by the individual applicant who wishes to join in karate classes or by the karatekas parents if they are juniors duly signed by the individual candidate and their parents.
The instructor of the concerned dojo to receive,sign and to keep it in his custody for his records to know when his student admitted in his dojo and to know how long he practiced in his dojo.
(D) – CLUB / DOJO / DISTRICT / STATE AFFILIATION FORM
Registration form
Is used by instructors who are willing to join with us via
1. Individual affiliation.
2. Dojo/ Club affiliation
3. District affiliation
4. State affiliation
have to be duly filled with their resume attaching a copy of their dan grade details.
Which will be duly considered by the SIKA admin committee
(E) – INSTRUCTORS CHART FOR COLOR BELTS
Instructors chart used by the Dojo/Club/ branch/district and state incharges to fill the details that how many color belts and brown belts are attending the grading at one time.
It’s a form of consolidated details of grading attendance at one time.
Say for example 100 karatekas attending grading they will submit individual kyu grade 100 forms.
Instructors keeping them for their records and sending the consolidated details only to the head quarters ie
Instead 100 forms only 2/5 sheets.
(F) – INSTRUCTOR’S CHART FOR BROWN BELT GRADING
Instructors chart used by the Dojo/Club/ branch/district and state incharges to fill the details that how many color belts and brown belts are attending the grading at one time.
It’s a form of consolidated details of grading attendance at one time.
Say for example 100 karatekas attending grading they will submit individual kyu grade 100 forms.

(G) – INSTRUCTORS’S CHART FOR BLACK BELT GRADING
In some style
They use strips according to the grade.
In some styles
They use white and red mixed belts for 7th to 9th dan.
For 10th dan they use Red belt
But in Shotokan we use only black belts for all ten dan/degrees.
